Eight organizations across the state are splitting $320,000 to help people with chronic health conditions stay healthy and in their homes.

The money comes from the Maine Health Access Foundation.

The Thriving in Place program supports groups as they work to keep people out of nursing homes and hospitals.

Among the local organizations getting some of the money, the Charlotte White Center, based in Bangor; Penobscot Community Health Center and Bucksport Bay Healthy Communities Coalition.
